Prescription Benefit Manager
Your prescription benefit manager is Optum Rx through March 31, 2018.
Prescription Programs Available
Optum Rx provides a retail, mail order and specialty drug program.
Mail Order Program
To receive your prescriptions through the mail, contact the Optum Rx Mail Order Pharmacy at 800-562-6223, or complete the Optum Rx Mail Order Form and mail it to:

Be sure to include your original prescription with the form. You may also find the information on their website at www.optumrx.com.
Drugs that Require Prior-Authorization from Optum Rx
There are certain drugs that require prior-authorization from Optum Rx. To view a complete listing, please click here. If your newly prescribed drug (i.e., not a current prescription that needs to be refilled), your provider must contact Optum Rx at 1-800-711-4555.
Prescription Drug Exclusion List
If your prescription drug appears on the Exclusion List, it will not be covered by your prescription drug plan. You are not required to change drugs, however, if you choose to continue using this medication, you will pay the full cost even if you received a prior authorization in the past. This does not mean you should stop taking your medication. We recommend you talk to your doctor to discuss alternative medication options.
Optum Rx ID Card Request
To request an Optum Rx identification card, click here. Be sure to include your name, Unique Identification Number (UID) or Social Security Number and current address in the email.
